Procedure for Availing Concessions and Accommodations and, applying for the UDID Card for SLD

Procedure for Availing Concessions and Accommodations and, applying for the Disability Card (UDID Card), for Specific Learning Disability (SLD) from the Government across all Boards (Samacheer Kalvi, CBSE, CISCE, Cambridge/IGCSE) in Tamil Nadu.

Note 1: For those living outside Chennai, procedure is similar. You have to approach the government hospital in your region.

Note 2: Disability card/UDID card for SLD will be given by the government hospital only after the child is 18 years. A child is considered to have SLD only if they have 40% benchmark disability and IQ is above 84.

It is important that the child gets an assessment done in a government hospital at the earliest (6 years and above). It becomes easy when the child requires concessions and accommodations for the board exams when the child is already enrolled into the government system.

Procedure in Chennai

1. Children below 12 yearsGovernment Children’s Hospital, Egmore
Go to the back gate (not the main entrance) and enquire at reception for assessment for Specific Learning Disability. You will be directed to the correct department. Please take your child with you.

Documents required

  • Copy of the child’s Aadhar card
  • Copies of all assessments done for SLD
  • Passport size photo of the child – 2 nos.

Procedure

  • You have to buy a notebook (available on campus).
  • Hospital will register your child.
  • Registration number and hospital seal will be affixed on the notebook.
  • Doctors will note all details including assessments in the notebook.
  • You have to carry the notebook every time you visit the hospital. Please keep the notebook until your child completes class 12.
  • Once the assessment procedure is complete, the hospital will give a certificate which is valid for either two or three years.
  • The child has to go for a reassessment at the end of the year mentioned in the certificate.
  • The child will be assessed again, and issued a fresh certificate with improvements/difficulties mentioned.
  • Please file all the original certificates and submit only copies to the school.
  • The child can go to the government children’s hospital until 12 years of age.
  • Once the child completes 12 years, you have to go to the Medical Board of any one of the following hospitals: Institute of Mental Health (IMH) Kilpauk, IMH Ayanavaram or Government Hospital Royapettah

2. Children above 12 years and up to 18 years – Institute of Mental Health (IMH) Kilpauk, IMH Ayanavaram, Government Hospital Royapettah – Medical Board

Note: Once the child completes 12 years you have to go to the Medical Board in any of the above-mentioned government hospitals.

First time assessment of a child above 12 years in a government hospital

Documents required

  • Letter from the school principal, on school letter head with signature and seal, stating: class child is studying in, difficulties the child has in academics, kind of accommodations required.
  • Letter from the parent addressed to the dean of the hospital stating the academic difficulties of the child and requesting accommodations for the same. Accommodations in the parent’s letter should be the same as in the school’s letter.
  • Copies of SLD assessments.
  • Aadhar copy
  • Passport size photographs of the child – 2 nos.

Procedure

  • Go to the Medical Board and submit your documents.
  • Date and time for assessment will be allotted.
  • It will take 5 to 6 sittings. Please be patient.
  • The entire procedure will take up to two months. Please begin early to avoid last minute rush.

3. Assessment of children above 12 years who are already enrolled in the government hospital system

If the child is already enrolled into the government hospital system below 12 years, then the child may go to the above-mentioned hospitals above the age of 12 as a routine process. This is valid only if the child has been regularly reassessed in the Government Children’s Hospital Egmore, until the age of 12 years.

4. Accommodations/concessions for class 10 and class 12 board exams

Please follow procedure in item no. 2.
The state government has become very strict in issuing concessions/accommodations certificates without prior assessments in government hospitals. It is important to build a case for the child as early as possible in order to be eligible for accommodations.

  • UDID Card for SLD
    The child should be 18 years.
  • Go to the government hospital that did the last assessment for the child.
  • Apply for the UDID card in that hospital.
  • Aadhar copy.
  • Copies of SLD assessments.
  • Passport size photographs of the child – 4 nos.
  • The hospital will do a final assessment and issue the card.
  • It will take up to 6 months for the card to be delivered by post to your home address.
